Your First Health Record

Start a simple, private health record you own and control.

Suggested duration: About thirty minutes, plus one week of logging

  1. Lesson 1: What to Write Down

A health record is a place to keep notes about your health. It helps you remember what happened, spot patterns and share clear facts when you want to.

You do not need to write down everything. Start with a few simple things.

Daily habits: how much water you drank, what you ate, how you moved and how you slept.

How you feel: your energy, your mood and your stress level. You can use simple words like great, okay or low.

Goals and wins: your small goal for the week and any wins, big or small.

Questions: anything you want to ask your doctor or care team next time.

Important basics: things like allergies, medicines you take and the names of your doctors. These can be helpful to have in one place.

Keep your notes short. A few words or a check mark is enough. The best record is one you actually keep.

PreFormance ME gives you a private place to store your notes. You own your record. You choose what goes in it and who can see it.

Over time, your record becomes a story of your health journey. It shows how far you have come.

    Action step:

Write down three things about your day: water, sleep and mood.

  2. Lesson 2: A Simple Food and Sleep Log

Food and sleep are two big pieces of how you feel. A simple log helps you see how they connect.

A food log is a list of what you eat and drink. You do not need to measure or count anything. Just write down the basics, like "eggs and toast for breakfast" or "chicken salad for lunch." You can also note how you felt after eating.

A sleep log tracks your rest. Write down what time you went to bed, what time you woke up and how rested you feel, using simple words like great, okay or tired.

Try to fill in your log at the same time every day. Many people do it at night before bed, or first thing in the morning.

Keep your log for at least a week. Then look back. Do you notice anything? Maybe you sleep better on days you eat dinner earlier. Maybe you feel more energy on days you eat more whole foods.

These patterns are clues. They help you learn what works for your body.

Your PreFormance ME record is a great home for your log. It keeps your notes in one private place, ready when you need them.

Remember, the goal is to learn, not to judge yourself.

    Action step:

Start your food and sleep log today with tonight's bedtime.

  3. Lesson 3: Sharing with a Provider

Your health record can be very helpful when you visit a doctor, nurse, dietitian or trainer. It gives them clear notes instead of guesses.

Before a visit, look over your record. Write down any questions you want to ask. It is easy to forget them once you are in the room.

Pick what to share. You do not have to share everything. Choose the notes that matter for this visit, like your sleep log or a list of your medicines.

Bring your notes with you, or have them ready on your phone. Your provider may spot patterns that help them understand you better.

Be honest. Your provider is there to help, not to judge. The more clearly you share, the better they can help you.

Listen and take notes. After the visit, add what you learned to your record. Write down any next steps your provider gives you.

Remember, PreFormance ME is a learning tool. It is not a doctor. Your provider is the right person to give advice about your own health, medicines or care.

With PreFormance ME, you decide what to share and when. You stay in charge of your record, and your provider gets the information that helps.

    Action step:

Write down one question you would like to ask at your next checkup.

  4. Lesson 4: Keep It Private

Your health notes are personal. They belong to you. That is why privacy is a big part of PreFormance ME.

Our slogan is "Predict. Perform. Private." Here is what it means. Predict means you use your notes to notice patterns and plan ahead. Perform means you use what you learn to feel and do your best. Private means your record stays yours.

Here are some easy ways to protect your health information. Use a strong password that you do not use anywhere else. Do not share your password, even with friends. Lock your phone and computer when you are not using them. Be careful about posting health details on social media.

Think before you share. Ask yourself who needs to see this and why. You can share just the part that matters, like one week of sleep notes, instead of your whole record.

Be careful with apps and websites. Before you put health information anywhere, check how it will be used.

Keeping your record private helps you feel safe being honest in it. And honest notes are the most helpful notes.

Congratulations! You finished Your First Health Record and all six PreFormance ME beginner courses.

    Action step:

Check that your phone or computer has a lock screen turned on.
